5.              The child’s full legal name (for each child)

6.              The child’s birth date  (for each child)

7.              How you heard about us

8.              Which clinic you would like to attend

 

We will call you to confirm your appointment.

The following dates and times are currently available:

6/05/08-           Thursday morning 10:00-12:30
6/16/08-           Monday evening  6:00 - 8:30

There will be NO clinics between June 17th and July 16th. 

Thursday morning clinics will resume on July 16th

Monday evening clinics will resume on July 21st
